Booking Appointments
We support our staff. They want to assist you to book, cancel, or change an appointment. The telephone will be busy usually from 9:00 to 10:30am. If you do not need to be seen today, then please call AFTER 10:30am. If you need to be seen today, then call as early as you can (starting at 9:00am). We request that our staff be courteous and patient – we expect that you will also be courteous and patient as well.

We now, after patients sign a consent, will communicate to patients via email  - but we do not accept e-mail sent to us.  Please CALL our office with any questions.

Fasting blood work
If you are having a fasting blood fasting blood sugar, or your physician has asked you to fast for your blood tests --- you should not EAT or DRINK anything for at least 12 hours before your test is done. Fasting blood work is ONLY done in the morning.
Health Card updates

Please call 905-833- 5381 ext # 3 if there is any updated information about your Health Card - especially a new version code.

If your Health Information Card (HIN) is damaged, lost, or stolen please arrange to have the card replaced.
This can be done at one of the service ontario locations listed below or Service Location finder - details about renewing your card

Anyone over 15 1/2 years old - up to 80th birthday will need a photo ID Health Card on renewal / replacement of their card.

Please note: our receptionist takes her break between 12:30pm and 1:30pm.
Our phones will not be answered and there will not be any staff at the receptionist desk.
If you need to pick up a document - please avoid the 12:30-1:30pm break period.

 

Lab

Our office lab will be closing as of Aug 22, 2012

We will no longer be taking blood samples at our office. We very much prefer that you use one of the 3 listed laboratories - as we will receive the results electronically (and not have to wait for mailed reports). This is especially true for INR tests - where waiting for a mailed report is NOT an option.

Nurse (RN) visits

Daytime visits (except when associated with a physician visit) require a booked appointment - for example:

  • 24 hour blood pressure monitor (2 visits, 24hr apart, both require appointment)
  • Suture or staple removal
  • BpTru (6 minute blood pressure check)
  • Immunization administration
  • Injections (such as B12, Testosterone, and others)
  • Allergy injections
  • TB test (and interpretation 48hr later)
  • Memory test administered by our nurse

Please note:  Our nurses no longer take blood, except under special circumstance.

Evening visits (Monday/Tuesday/Thursday) 5:30-7:30pm do not need a booked appointment, the RN will see you a first come - first served basis.

Office courtesy

Our physicians expect our staff and patients to show patience and courtesy when dealing with each other. Please respect our staff - they are doing their best to help you. Abuse of our staff will not be tolerated.

Our busiest telephone times are between 9am and 10:30am. If your call is not related to being seen on the same day, then try waiting until later in the morning (or after 1:30pm) to call our receptionist.  Our receptionist will try to answer your telephone call as quickly as possible - but she is also greeting patients who are arriving for their appointments, faxing prescriptions,  managing charts, and notifying staff of patient arrivals.

Prescription renewals

Medication RENEWAL:
We request that you telephone your pharmacy with the request for the renewal. We expect that your pharmacy will FAX the request to our office for review by your physician. Our turn around time is usually same day - but there are occasions when it may take 2 or 3 weekdays to renew a medication.

Please try to AVOID renewing your medication on the day you run out !

Referrals

Who to contact for more information

For information about the status of your REFERRAL: please call directly to (905) 833-5381 ext #1  (9:00am to 12:30pm, 1:30pm to 4:00pm)

Referral process explained:

Your physician will create the referral and give it to our secretary. The referral is faxed to the specialist's office (the standard method).
Our staff then wait for a return fax giving us the date/time for your appointment (except in some cases when the specialist's office contact you directly).

For CT or MRI scans - we arrange a referral, but (in almost every instance) the hospital will call YOU directly with the appointment date + time.

In MOST cases we hear back from the specialist's office in 2-3 weeks. However, we have a few specialists that will take 1 to 2 1/2 years to get back to us. This is the reality we work with. We have hundreds of referrals in action at any one time.  We will contact you when we know the details of your appointment.  If you wish to call to check the status, please wait at least TWO weeks before calling (after the referral is sent) - unless you have been given special instructions by your physician or our staff.

 

 

Transfer of Medical Records to a new Physician

We offer 2 choices:

 1)   at no charge, print out a one page summary (CPP i.e. cumulative patient profile)

or

2)   for $40 we will electronically capture the information in your chart (dating from mid 2008 to the present) and send an encrypted (password protected) flash drive (thumb drive) to your new physician. We separate categories such as Lab reports, Xrays, Consultant reports, the CPP so that your new physician will be able to find the information that they need.
